Plan and implement outreach, case management, advocacy, linkage and education to consumers seeking prenatal and postpartum support
services, including facilitated insurance enrollment.
Graduate from an accredited college with Bachelor's Degree in Social Work or other Human Services field, or Licensed RN. Experience in case
management, maternal child health, and patient education wanted. Valid New York State drivers license required. Facilitated insurance
enrollment experience preferred. Strong communication skills required. Proficient computer skills a Microsoft Office experience preferred.
Demonstrated ability to teach teens, young adults, and adults. Ability to work independently and use sound judgement. Program management
experience helpful. Car seat safety technician certification required or must be obtained within first year of employment. Proven work history as a
self-starter working autonomously to meet established objectives, set priorities and resolve problems. Ability to effectively multi-task with proven
flexibility to meet changing priorities and demands daily.
